千代田区麹町、麹町駅と半蔵門駅のちょうど間にある整形外科クリニック
“One Clinic 麴町(ワンクリニック麹町)” 院長の栗本です。
暖かい日もあるものの着実に冬の足音が聞こえてくる今日この頃ですが、皆様いかがお過ごしでしょうか。
今回は、タイトルにもある通り「整形外科と形成外科の違いとは?」について書いてみたいと思います。
私が「よし、(救命救急医ではなく)整形外科医になるか!!」と決めた頃からよくある会話で
相手:何科ですか?
私:整形外科です
相手:あー、美容?
私:いや、骨折や人工関節・背骨等の手術をする方です
相手:あ、美容と骨折って別の科なんだ?
私:はい、整形外科と形成外科で別ですね
というものがありました。
恐らく「美容整形」という言葉がある事・”整形外科”と”形成外科”の響きが近い事が原因かと思います。
(美容に携わるドクター達の専門は形成外科だけでなく皮膚科や麻酔科等多岐にわたりますが、やはり形成外科のドクターが多いように思います!(^^)!)
整形外科と形成外科は、扱う疾患や治療方法に違いがあります(一部共通するものもあり、これが更に違いを分かりづらくさせています。。)
ざっくりですが整形外科は、骨や関節、筋肉、神経などの運動器の病気を扱う科です。具体的には、以下のような疾患や症状の治療を行います。
・骨折や脱臼などの外傷
・骨や関節の炎症や変形
・変形性関節症や脊椎(腰痛、頸椎症など)の変性疾患
・スポーツ障害
・(一部)リウマチ
それに対して形成外科は、身体表面の傷や変形、欠損などを扱う科です。具体的には、以下のような疾患や症状の治療を行います。
・熱傷
・外傷後の瘢痕
・先天性異常(口唇口蓋裂、小耳症など)
・(救命分野では)血行再検
・皮膚の欠損
・美容目的の治療
手の治療や植皮は整形外科も形成外科も行うため、どちらに受診すればよいのかわかりづらいこともあると思います。いつでも遠慮なくご相談ください。

This time, as the title says, I would like to discus “What is the difference between Orthopedic surgery and plastic surgery?” I would like to write about the following.
It has been a common conversation since I decided to become a Orthopedic surgeon (instead of an emergency physician).
Other person: What department are you in?
Me: Orthopedic surgery.
Other person: Oh, cosmetic?
Me: No, the one who operates on fractures, artificial joints, spines, etc.
The other person: Oh, cosmetic and broken bones are treated in two different departments?
Me: Yes, they are separate, orthopedics and plastic surgery.
The other person: Yes, they are two different departments.
Perhaps it is because the word “cosmetic surgery” exists and “Orthopedic surgery” and “plastic surgery” sound similar in Japanese.
(Doctors involved in cosmetic surgery specialize not only in plastic surgery but also in dermatology, anesthesiology, and a wide range of other fields, but it seems that many of them are plastic surgeons! (^^)!
Orthopedic and Plastic Surgery differ in the diseases they treat and the way they treat them (some of them are the same, which makes the difference even more difficult to understand).
Roughly speaking, orthopedics is a department that deals with diseases of the musculoskeletal system, such as bones, joints, muscles, and nerves. Specifically, it treats the following diseases and symptoms
Traumatic injuries such as fractures and dislocations
Inflammation and deformation of bones and joints
Osteoarthritis and degenerative diseases of the spine (lumbago, cervical spondylosis, etc.)
Sports injuries
(Some) rheumatism
In contrast, plastic surgery is a department that deals with wounds, deformities, and defects of the body’s surface. Specifically, the Department of Plastic and Reconstructive Surgery treats the following diseases and conditions
Burns
Scars after trauma
Congenital anomalies (cleft lip and palate, microtia, etc.)
Blood retracing (in the lifesaving field)
Skin defects
Cosmetic treatments
Since both plastic and Orthopedic surgery perform hand treatments and skin grafts, it is sometimes difficult to know which one to see.
